Appendix: Leader Forms & Agreements
Below are forms and agreements Relationship Toolshop® Leaders might need. In the left-hand menu, click on the name of the form you want and then click “download” to save the file to your computer before filling it out. (See bottom of page for submission guidelines.)

- To submit a form in Word format: type into the form and save it with your name in the filename before submitting.
- To submit a form in PDF format: print, complete, sign, then fax (937-748-4541) or scan and email the form to SPcommittee@RelationshipToolshop.com.
